This assessment was developed by Dr. Paul R. Belcher, D.Min, adapting a spiritual-impact assessment tool originally created for military chaplaincy use. It helps emergency responders, law enforcement, ER/trauma medical staff, and military members and veterans reflect on how traumatic experiences have affected them physically, emotionally, and spiritually, and connects those who want it with support through SoulDocs. This is a self-reflection and screening tool, not a diagnostic instrument, and is not a substitute for evaluation or treatment by a licensed professional.
Participation is completely voluntary and you may stop at any time. No name, email, or other identifying information is required. Only demographic information is collected, and only in de-identified, aggregate form. At the end you may optionally share contact information if you’d like to learn more about SoulDocs’ support program.

Spirituality is a sense of connection that gives meaning and purpose to a person’s life — the deepest part of you that gives a sense of who you are, why you are here, and what your purpose is. Religious means participation in organized denominational gatherings (e.g. Protestant, Catholic, Muslim, Jewish, Hindu, Buddhist, or other faiths) with traditions and rituals that strengthen faith.
